Personally I think 3 factors. His long persuit of berahino and then not even playing him or not managing it professionally ( although we don't know the full story)
Sticking to his stubborn belief in 3 at the back, when clearly we don't have good enough wing backs for this formation to work
Also placing all this summers transfer hopes on getting Fabian Delph... didn't work and as a result ended up with fletcher as back up

It's the beard for me. Mark himself clearly felt it made him look wind swept and interesting and at least one lady reporter positively swooned at it, but those close to him either looked grim faced or struggled to suppress a snigger.

Shitting himself in April 2016 and refusing to play with more than 1/2 flair players. Haven't tried to play football since.
If we're going for 3 mistakes then add on:
Not signing a right back after Johnson turned to shit. 442.

The backup goalkeeper situation at the end of 15/16 could be considered the start of our rot. I’m still convinced the seemingly desperate signing of Grant, and his subsequent terrific form during the first half of 16/17, was more a stroke of tremendous luck rather than expert judgment.

Zonal marking, no battling defensive midfielders, not getting a Huth replacement in Huth's mould.

awful recruitment.:.
When we had a Spine of Pulis's grafters which allowed Sparky to sprinkle in some flair we looked real good, beat some very good teams and finished well in the league.... but as the pulis players have diminished he's not acquired many who would run themselves into the ground for us, instead preferring to spunk great swathes of cash ( with some exceptions) on mediocrity, the aged or the talentless.
He's not maintained the balance of what he initially built - a good side, we now look a bit brittle and honestly, a bit shit.

Zonal marking, no battling defensive midfielders, not getting a Huth replacement in Huth's mould. This. Defensively our managerial/coaching team appear not to have a clue.

Buying too many European show ponies I still to this day can't understand why Hughes after the way he played signed players with all the balls of your average eunuch

1) Never building from the back when he first arrived 2) inside forwards preferring guile over pace & natural wingers 3) insisting on a shit boring possession based slow tempo football 4) not doing homework on big signings (laziness, poor management) 5) resigning a contract for himself 6) selling huth
Fuck it stinks, get rid of the smell

Hughes biggest & most costly mistake was early on selling Huth to Leicester was criminal for lots of reasons. He was the one player who held us together scored the odd important goal & Ryan has never been the same player without him (injuries aside). I also think he made a very large contribution to Leicester winning the title against all the odds I bet Hughes version of this would be very different.

Playing Bojan at Rochdale. He could of done it very next game though. You can't not play players so they don't get hurt what is the point in signing them

Deciding he was staying on for a 5th season. Yes I think managers just get stale and loose their way after a few years, only Ferguson has ever spent many years at a club and not seen performances decline.
