What does an "option of a further 12 months" actually mean? Is it a guarantee that a further year will be activated (or offered?) if certain conditions are fulfilled? What happens if it turns out he doesn't want another year for some reason? I would of thought it would be an optional extension available to the club (i.e. if the management still think he has enough to offer they can activate the clause). May be based on appearances as well like Jamo said. I don't see how the club is the beneficiary of such a clause though. Why not just offer him another year after this year without making any promises now? At 34 he's not likely to have a better offer at that time, surely? I suppose it's an added incentive for the player to sign now, but again, I can't imagine he's swamped with other offers...
