I think not acting decisively after a spawny win at Watford will be the one that haunts us the longest. See also - not acting decisively after a spawny win at Swansea. While the sentiment might be true, neither win was spawny.

You can draw your own conclusions about that. The argument is always that the stand was not insured and that's not true. The difference in the valuation of the repairs, between the insurance assessors and the contractor was massive. Who actually actually carried out the repairs? Does anyone know?

Deciding not to sign Demba Ba. If memory serves me right he’d all but signed, there was even a photo of him in the stoke strip but something showed up in his medical ? After that he went to Newcastle and proceeded to bang in a shed load of goals. The myth of the exploding knee saga

My two penneth for what got us to this point. So maybe not in our history but in this brief history of time.
Signing Crouch and Palacios - that was the first really bad long term decision the club made in the PL.
Getting rid of Huth.
Not replacing the pace of Moses.
Replacing Fonz with Van Ginkel and then being desperate so we needed Imbula.
Signing Berahino and Wimmer for the money we did. Not even necessarily the players but the money we put down for them was obscence.
Not sacking Hughes after the Arsenal home defeat in the 16/17 season.
Not having a proper manager ready for when they did sack Hughes.
When we got relegated, not getting rid of Scholes and Cartwright and having a full and proper clearout from the board down.
Not selling Butland and Allen when we were relegated. And that isn't hindsight, they were the two I wanted go from the off.
The last transfer window.

Managerial appointments and (delayed) sackings have been huge, and frequently bad, decisions over the past few years, but I guess it is easier to criticise the board of 1947 than the one of 2019!

Appointing Paul Lambert at a time we needed someone more up to speed who could get the ground running & with greater experience of a Premier League relegation battle. I know on reflection he tried his best & there were some unlucky incidents ie the Adam penalty miss & Butland juggling the ball into the net at Leicester but that day he got appointed I really did lose all hope of us staying up & my initial suspicions weren't wrong.
& on the Lambert subject. Was sacking him off after that final game of the season & not giving him a season to see how he did in the Champo prove costly too? I'll admit I wanted a change at the time but maybe just maybe he knew enough about the club & squad that he'd of faired better last season than the efforts Rowett & Jones mustered up over a season?! Ipswich kept faith with him after a relegation & they appear to bossing League one right now & look as though they'll be gaining promotion back this season if they carry on as they are doing.
It's all What if's and maybe's!

I would say that the decision not to sign Demba Ba in 2011, for alleged medical reasons, was a fairly critical moment in Stoke's Premier League history. We might have got a fairly useful striker who would have helped with a sustained challenge in the top ten, potentially being able to offer double figure goal returns in a season. We turned down the chance to acquire his services on medical advice and he went on to complete short but reasonably succesful spells at West Ham, Newcastle and Chelsea, despite being written off by Stoke.

As far as I remember we made a big thing about our 'due diligence' around that time, evidently not in this case. Palacios was the main man in the deal as I recall, Crouch was the makeweight. Not letting Zonz run down his contract but cashing in for 7 million, a large chunk of which went on a loan fee to Chelsea was a pisspoor decision.
